Over-the-Counter Medications(Required)

Over-the-counter medications provided by the parent/guardian will be administered by the nurse or her trained designee according to manufacturer’s directions unless otherwise directed in writing by a physician.

On an occasional basis and only with the parent’s signature to the permission option below, the nurse or her designee will administer over-the-counter medications according to the manufacturer’s directions that are provided by the School District. No over-the-counter medication will be given to a student if this form has not been signed. Verbal or telephone consent is not sufficient. If repeated requests are made, the student will be referred to his/her parent to determine if a visit to the doctor is necessary. The school district reserves the right to revoke this service at any time.

A specific list of medicines provided by the district can be found at this link. You may also specifically list medicines you do not want the school to provide later in the form.

Districts in the state of Arkansas bill Medicaid for re-payment for specific student screenings, such as hearing and vision. Law requires parent/guardian permission for our district to release student screening information to state agencies for billing purposes. This will be of no cost to you. Funds obtained from these screenings are designated for the purchase of health supplies, health equipment, nursing costs, and nursing professional development.

School Nurse Office Policies and Information(Required)

Kids must stay home if they have fever, vomiting, or diarrhea, and must stay home for 24 hours after the symptoms resolve (without the aid of medication). PLEASE DO NOT send a sick child to school. They will be sent home. If a child vomits at school, it must be verified by an adult.

Over-the-counter medications will only be dispensed between 9:00am and 2:00pm at the nurse’s discretion and will be dispensed according to manufacturer recommendations. Only ONE dose will be administered during a school day unless a doctor orders otherwise.

Unless authorized to self-administer or otherwise authorized by this policy, students are not allowed to carry any medications, including over-the-counter (OTC) medications or any dietary supplement or other perceived health remedies not regulated by the US Food and Drug Administration while at school.

Medications, including those for self-administration, shall be brought by the parent or legal guardian to the school nurse. Medications can be brought to the school nurse by the student if accompanied by a written authorization from the parent or legal guardian. The medication must be in the original container and be properly labeled with the student’s name, the ordering provider’s name, the name of the medication, the dosage, frequency, and instructions for the administration of the medication (including times). Additional information accompanying the medication shall state the purpose for the medication, its possible side effects, and any other pertinent instructions (such as special storage requirements) or warnings. Paperwork verifying the quantity of medication (s) must be signed by each person. If there is a change in dosing, a doctor’s order must be sent to the nurse’s office before the change can be administered. Medication belonging to another member of the family will not be administered. (Once a day and twice a day dosing of meds are NOT administered at school unless ordered for lunchtime dose.) If an antibiotic is required to be taken during school hours, a separate school bottle must be brought to the nurse’s office. Meds will not be sent home daily with a child.

NO narcotic (pain medication) is allowed at school. If your child requires high-dose pain meds, they need to be excused from school until normal activity can be done without the use of such meds.

Teachers must always be aware of the student’s location in the building or on campus, so we require that all kids have a nurse referral form or a call from the teacher to go to the nurse's office. Kids may not come between classes without first checking in with their teacher, with the exception of an emergency circumstance. If a child is coming to the nurse’s office for daily medication or rescue medication (inhaler or epi-pen), a pass is not required.

The school nurse will notify the parent IF a student needs to go home, if there is a concern about their well-being, or if something needs to be monitored at home. Students are not allowed to call parents and report being sick from their cell phones, and they must come to see the nurse if they complain of illness at school. Such absences will be marked as a parent note or unexcused.

The fact that a student with a completed consent form on file is allowed to carry a rescue inhaler, auto-injectable epinephrine, diabetes medication, stress dose medication, or combination does not require the student to have such on the student’s person. The parent or guardian of a student who qualifies under this policy to self-carry a rescue inhaler, auto-injectable epinephrine, diabetes medication, stress dose medication, or any combination on the student’s person shall provide the school with the appropriate medication, which shall be immediately available to the student in an emergency.

A student is prohibited from sharing, transferring, or in any way diverting his/her medications to any other person.
Please make sure the office has updated numbers and information at all times should anything happen through the school day and a parent/guardian needs to be notified.

Objection to Publication of Directory Information

Care and Usage of School-Provided Computer(Required)

Like any other school property that is checked out to a student to use for educational purposes, the issued computer should be safeguarded and treated with care. The following rules and regulations are to be adhered to at all time by the student who has legally taken possession of a school computer that has been assigned to him/her:

You will be assigned a school computer, which you and your parent/guardian will sign for upon taking possession of the device.

Your parent/guardian has been offered a protection plan (see details in “Chromebook Manual”) for a annual cost of $25.00 and will initial to accept or decline for this protection plan below.

You will be required to follow all rules and regulations of your school, including the “Computer Use Policy” described in the student handbook. Furthermore, the use of your school computer for cyberbullying or any other illegal activity is strictly prohibited and will be dealt with as per your school’s disciplinary policy.

You are required to bring your computer to your classes each day. It is your responsibility to make sure that your computer is completely charged prior to the beginning of school.

Computer Protection Policy

Please understand that the computer protection plan does not cover intentional damage or negligent loss/theft of a device.

Student Electronic Device & Internet Use Agreement(Required)

The West Fork School District agrees to allow the student identified above (“Student”) to use the district’s technology to access the Internet under the following terms and conditions which apply whether the access is through a District or student-owned electronic device (as used in this Agreement, "electronic device" means anything that can be used to transmit or capture images, sound, or data):

Conditional Privilege: The Student’s use of the district’s access to the Internet is a privilege conditioned on the Student’s abiding by this agreement. No student may use the district’s access to the Internet whether through a District or student-owned electronic device unless the Student and his/her parent or guardian have read and signed this agreement.

Acceptable Use: The Student agrees that he/she will use the District’s Internet access for educational purposes only. In using the Internet, the Student agrees to obey all federal laws and regulations and any State laws and rules. The Student also agrees to abide by any Internet use rules instituted at the Student’s school or class, whether those rules are written or oral.

Penalties for Improper Use: If the Student violates this agreement and misuses the Internet, the Student shall be subject to disciplinary action. Repeat offenders may face more severe punishment. Each school principal shall be responsible for establishing reasonable regulations for the enforcement of this policy.

Misuse of the District’s access to the Internet includes, but is not limited to, the following:
1. Using the Internet for other than educational purposes;
2. Gaining intentional access or maintaining access to materials that are “harmful to minors” as defined by Arkansas law;
3. Using the Internet for any illegal activity, including computer hacking and copyright or intellectual property law violations;
4. Making unauthorized copies of computer software;
5. Accessing “chat lines” unless authorized by the instructor for a class activity directly supervised by a staff member;
6. Using abusive or profane language in private messages on the system; or using the system to harass, insult, or verbally attack others;
7. Posting anonymous messages on the system;
8. Using encryption software;
9. Wasteful use of limited resources provided by the school including paper;
10. Causing congestion of the network through lengthy downloads of files;
11. Vandalizing data of another user;
12. Obtaining or sending information that could be used to make destructive devices such as guns, weapons, bombs, explosives, or fireworks;
13. Gaining or attempting to gain unauthorized access to resources or files;
14. Identifying oneself with another person’s name or password or using an account or password of another user without proper authorization;
15. Invading the privacy of individuals;
16. Divulging personally identifying information about himself/herself or anyone else either on the Internet or in an email unless it is a necessary and integral part of the student's academic endeavor. Personally identifying information includes full names, addresses, and phone numbers;
17. Using the network for financial or commercial gain without district permission;
18. Theft or vandalism of data, equipment, or intellectual property;
19. Attempting to gain access or gaining access to student records, grades, or files;
20. Introducing a virus to, or otherwise improperly tampering with the system;
21. Degrading or disrupting equipment or system performance;
22. Creating a web page or associating a web page with the school or school district without proper authorization;
23. Providing access to the District’s Internet Access to unauthorized individuals;
24. Failing to obey school or classroom Internet use rules; or
25. Taking part in any activity related to Internet use which creates a clear and present danger of the substantial disruption of the orderly operation of the district or any of its schools;
26. Installing or downloading software on district computers without prior approval of the technology director or his/her designee;
27. Storing files that are not school-related in the home directory;
28. Connecting personal laptops to the school network; or
29. Connecting flash drives or other data storage devices for activities not school-related and/or without the supervision of school personnel.

Liability for debts: Students and their cosigners shall be liable for any and all costs (debts) incurred through the student’s use of the computers or access to the Internet, including penalties for copyright violations.

No Expectation of Privacy: The Student and parent/guardian signing below agree that if the Student uses the Internet through the District’s access, that the Student waives any right to privacy the Student may have for such use. The Student and the parent/guardian agree that the district may monitor the Student’s use of the District’s Internet Access and may also examine all system activities the Student participates in, including but not limited to e-mail, voice, and video transmissions, to ensure proper use of the system. The District may share such transmissions with the Student’s parents/guardians.

No Guarantees: The District will make good faith efforts to protect children from improper or harmful matter which may be on the Internet. At the same time, in signing this agreement, the parent and Student recognize that the District makes no guarantees about preventing improper access to such materials on the part of the Student.

Building & Grade Specific Information

Title I, Part A School Compact & Right to Know(Required)

West Fork Elementary School and West Fork Middle School are both Title I, Part A Schools meaning they both receive funding from the Federal government, in part to help close the achievement gap between disadvantaged and minority students and their peers. Because these two schools receive Title I, Part A funds, both schools at the beginning of each school year must inform parents of each student attending the school of their right to request information about the professional qualifications of both the teacher and the paraprofessionals who teach and work with their children in an understandable and uniform format. Such requests should be directed to your child’s building principal in the form of a written letter.

School-Parent-Student Compact

The , and the parents of the students participating in activities, services, and programs funded by Title I, Part A of the Elementary and Secondary Education Act (ESEA), agree that this compact outlines how the parents, the entire school staff, and the students will share the responsibility for improved student academic achievement and the means by which the school and parents will build and develop a partnership that will help children achieve the State’s high standards.

Elementary and Middle school students should only purchase the computer protection plan.

Our online payment system requires that you know your student's Student ID number. Since your student does not yet have an ID, we will send a follow-up email after your student's registration has been processed. This email will contain your student's new ID and a link to purchase things like a computer protection plan and a parking pass.

If you do not receive this email, don't worry, you will be able to purchase in person at your school's office.

PowerSchool Registration

At this point, you have completed all of the annual district and state forms, and it is time to proceed to the required registration portion.

Please note that, you will be required to have a PowerSchool account. If you registered a student with us last year, you will already have an account, and you can use the forgot password option if required. If you did NOT register a student with us last year, you will need to create an account on the next page.

As part of this process, you will be asked to upload several documents. Please see the list below to know whether or not these are required.

Proof of Residency: This is required for new students or if the student has moved since your last registration. You may also submit this to the office in-person.
Court order (custody or otherwise) Document: Not required, but permitted
Parent Pickup Forms: Not required
Student's Shot Record: This is required for new students or from students with shots not reflected in our files. You may also submit this to the office in-person.
Medical Document: Not required, but permitted
Home Language Survey: Not required
Kindergarten Physical: Only required for incoming Kindergarten students.
504 Plan: Not required, but permitted
Agricultural (Migrant) Survey: Not required
McKinney Vento: Not required
